Prazosin is a hypotensive agent that is also an alpha-blocker.  The drug lowers blood pressure and has a beneficial effect on the heart with heart failure.  The main action of the substance is vasodilation.  Due to the expansion of veins and arteries, the drug reduces peripheral resistance and facilitates the work of the heart.

Pharmacological action of  Prazosin

Selectively blocks postynaptic alpha1-adrenergic receptors.  It is quickly absorbed in the gastrointestinal tract, the absorption rate does not depend on food intake.  Bioavailability is 50–85%.  In the blood, bound to proteins by 97%.  As a result of degradation, 4 metabolites are formed in the liver, which account for up to 25% of pharmacological activity.  T1 / 2 - 2-4 hours, with renal failure and in the elderly, it increases.  Excreted mainly by bile.

It has a pronounced hypotensive effect without a significant increase in heart rate.  Reduces total peripheral vascular resistance, expands capacitive and resistive vessels.  Significantly reduces pressure in the pulmonary veins and right atrium, moderate - pulmonary vascular resistance.  Taking the first dose often causes a sharp drop in blood pressure (especially in orthostasis and while taking diuretics).  After cancellation and subsequent administration within a week, a similar effect from taking the first dose is not observed.  Reduces myocardial oxygen demand, increases stroke volume during physical exertion.  It has a minimal effect on fluid retention in the body, does not increase body weight, does not change the filtration rate and renal blood flow, and reduces the resistance of renal vessels.  The peak of the hypotensive effect after a single dose is observed after 1–4 hours, the duration is up to 10 hours.  Both early (by 4–5 days) and late (by 12 months) tolerance can develop, which requires an increase in dose.  Favorably affects the lipid spectrum: the level of total cholesterol, triglycerides, VLDL and LDL decreases;  does not alter carbohydrate tolerance or uric acid metabolism.

 

Indications Prazosin

Contraindications Prazosin

Contraindications to the use of the drug Prazosin are:

 • Hypersensitivity to any components of the drug;

 • Heart failure with constrictive pericarditis;

 • Pregnancy

 • Breast-feeding

 • Age to 12 years

Dosage Prazosin

The drug Prazosin is taken orally before bedtime in a horizontal position (at the beginning of the course of treatment. The initial dosage is 0.5-1 mg. Then Prazosin is taken 2-3 times a day for 1 mg. Every 5-7 days of administration, you can switch from one dosage to a higher one (6-15 mg for 2-3 doses per day.) The daily maximum dose is 20 mg.
